They were unaware that their sugar was elevated until I screened them. Their A1C was elevated, and they were having symptoms of elevated blood sugar. The client did not have health insurance but MAP staff were at the outreach event. They signed them up and I called CommUnityCare to get them seen the same week. I also assisted the client with an appointment to get bus passes at the St. John Neighborhood Center. The client was given educational materials on prediabetes and nutrition. Client did see the doctor and was reluctant to start the medication. They were given medicine from Mexico and they wanted to try that medicine first but it was not helping. Discussed with the client their reluctance about taking the medication and they were just nervous about how it would make them feel. The client trusted me to try it and their A1C came down after a few months from 12 to 8. The client has changed their eating and drinking habits. They have begun exercising and seeing the doctor regularly. They are also coming in for screening from time to time and are still working on lowering their A1C. 2
